Youth-in Transition Services

Youth-in Transition Services

The Youth in Transition (YIT) Program provides support to youth/young adults who are currently living independently or who will be transitioning into independent living situations. Youth must be between the ages of 16- 24 years and residing in Lanark County and also be subject to a crown wardship, or eligible to receive other supports from Family Children Services of Lanark, Leeds and Grenville (FCSLLG). Examples of such are: Renewed Youth Supports, Continued Care and Support for Youth, and Voluntary Youth Service Agreement (VYSA)..

Client Referral Forms

Anyone can make a referral for a child or youth under the age of 18 – self, parents or guardians, family doctors, schools or other service providers – who feel, in discussion with the youth or family, that the child, youth or family would benefit from our services. A doctor’s referral is not required.
